Athletic Forms & Documents

The following forms must be completed in order for student-athletes to be registered as a member of any team at Stockbridge High School. Please note, you will need a parent/legal guardian to complete or submit these forms.

Each student-athlete must submit a current, valid physical to the athletic office each year. Athletic physicals are valid for 365 days/1 year after completion. It is best practice for student-athletes to obtain and submit a physical each summer before the school year begins.
